This is a direct care position that has the goal of keeping youth in their homes, schools, and communities by coordinating the necessary services for youth with complex behavioral health and multi-system needs within a High-Fidelity Wraparound model. The specific activities of the CME Care Coordinator include regularly meeting with youth and their families, developing child and family centered care plans, monitoring the care plans through regular scheduled multidisciplinary wraparound team meetings, and assisting youth and families/caregivers with developing or enhancing access to resources and natural supports.
CME Care Coordinator Job Duties:
- Build professional relationships with clients to assist them in achieving their goals.
- Regularly engage with youth and families on a community-based level in a culturally competent manner to promote the family’s voice and choice.
- Assess level of care using the Ohio Comprehensive Child and Adolescent Needs and Strengths(CANS) Assessment.
- Organize wraparound team meetings and maintain documentation of these meetings.
- Connect youth and families/caregivers to community resources, natural resources, and work collaboratively with members of the team.
- Links youth and families to specialized clinical resources to meet their treatment needs.
- Responds effectively and is able to access resources for clients and families in crisis situations.
- Works closely with youth and families to develop crisis and care plans that incorporate family voice and choice; regularly monitors and adjusts plans as necessary.
- Collects outcome information by administrating and collecting necessary forms from client and family.
- Participation in regular training regarding health equity/health disparities and cultural competence.
CME Care Coordinator Educational and Experience Requirements:
- Three (3) years of experience with a High School Diploma or Two (2) years of experience with an Associate’s or Bachelor’s Degree or One (1) year of experience with a Master’s Degree.
- Experience in children’s behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral health care field, providing community-based services to children and youth and their family or caregivers.
- Background and/or experience in one or more of the following: family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development preferred.
- Cultural knowledge of key populations that will be served preferred
