Mountain Park Health Center is looking for Licensed Professional Counselors (LPC) to join our well established and accredited team of providers. As a large Federally Qualified Health Center (FQHC) in Arizona, we are dedicated to sustaining and improving health by providing excellent, patient-centered primary care.
In this role as a Licensed Professional Counselor, you will coordinate with primary medical and behavioral health teams to ensure delivery of comprehensive care to patients. As a Licensed Professional Counselor (LPC), you will be working as an integrated team member in one of our primary care clinics. LPCs assess patients, create, and coordinate treatment plans, and oversee the ongoing treatment of assigned patients. LPC’s also can provide traditional counseling/therapy to the pediatrics and adolescent population.
Essential Functions:
- Provides brief, evidence-based, solution focused behavioral interventions with active development of psychotherapeutic/psychoeducational behavioral change planning for patients of Mountain Park Health Center.
- Provides quality clinical consultation to patients and providers regarding mental, behavioral, emotional, and physical health related issues.
- Provides screening and functional assessment of behavioral health issues for Mountain Park Health Center patients.
- Collaborates with medical providers, support staff, and operational managers to identify patients with medical, cognitive, behavioral, and/or psychological conditions that could benefit from behavioral health interventions, such as motivational interviewing, and provide these services in a timely manner within the clinic setting.
- Develops and implements specific clinical pathways for select populations.
- Facilitates groups to address biopsychosocial issues impacting medical issues.
- Assists with coordination of care and access to behavioral health agencies in the community to meet the patient’s behavioral health care needs with consistent, expeditious feedback to referring providers.
- Provides crisis triage and intervention.
- Helps with de-escalating situations with patients who are upset or frustrated.
- Maintains regular and predictable attendance.
- Performs other duties as assigned.
Qualifications:
Minimum Qualifications:
- Current licensure in the state of Arizona as a Professional Counselor.
- Must be eligible for credentialing and contracting with all insurers.
- Valid CPR card.
Preferred Qualifications:
- Training in Motivational Interviewing (MI).
- Training in Acceptance and Commitment Therapy (ACT).
