Adapt Integrated Health Care is looking for a Case Manager to join their Crisis team!

The Crisis Case Manager works with individuals diagnosed with mental illness to promote recovery, independence, and successful community living. This is accomplished through the provision a variety of treatment services including crisis intervention under the guidance of a therapist (QMHP) and/or supervisor, individual and group skills training, and development; case management services; coordination and monitoring daily structured activities of clients. Performs crisis intervention as necessary and provides consultation and training. Facilitates communication and correspondence with other Adapt staff, community partners, other treatment providers and peers. Position coordinates and collaborates with multi-disciplinary treatment teams and is responsible for work performed in assigned specialized program area (i.e., ACT, EASA, PSRB, .370, Jail Diversion, etc.).

Primary Responsibilities:

  • Provides case management, individual and group skills training and development, mentoring and crisis intervention as directed by a QMHP and documented in the Individual Service and Support Plan (ISSP).
  • Actively promotes the harmonious delivery of services through coordination of interventions in collaboration with treatment team members and foster providers achieved by ongoing communication and active problem solving around outcomes, barriers, and case observations.
  • Maintains and completes required records, reports, documentation as required by departmental policy and state and federal law.
  • Transports individuals, family members and other supports to/from services.
  • Participates in professional development through individual and group supervision, unit and agency staff meetings, workshops and Litmos tutorials.
  • Develops, modifies, and or obtains training programs and materials in consultation with supervisor and program manager.
  • Promotes peer professional development by sharing training materials and or training others in successful interventions.
  • Position supports crisis and specialized programs and works with law enforcement, the State and local hospital and CCO’s to coordinate clients’ re-entry into communities.
  • Respond to crisis calls within the community in coordination with the mobile crisis team, providing resources, safety planning and crisis interventions under the supervision of a QMHP.
  • Receives general instructions regarding the scope and approach to projects or assignments but is able to ask for and obtain specific procedures and problem resolution strategies when needed.
  • Participates in regularly scheduled supervision and meetings to ensure determinations and decisions made are in compliance with departmental goals, policies, and procedures.
  • On-going interaction, effective communication, and active collaboration with individuals and families receiving treatment services through Adapt Integrated Health Care, natural and community supports, agency partners, and other treatment providers.
  • Provision of education for treatment and prevention, and outreach efforts to specific individuals and the general public.
  • Emergency Response: This employee has been identified as a staff member expected to report for duty following activation of a public health emergency response plan.
  • Other duties as assigned by supervisor.

Qualifications


Required Qualifications:

  • Bachelor’s degree and two years’ experience on a social service and/or mental health setting or a satisfactory equivalent combination of education, experience, and training.
  • Must possess a valid Oregon Driver’s License.
  • Must be eligible to register as a QMHA through MHACBO.
