A Brief Overview
The Postpartum Continuity of Care Nurse Coordinator is responsible for contacting patients who are postpartum from giving birth on the Special Delivery Unit to assess their clinical status and provide support, guidance and resources regarding medical, mental health and lactation needs, scheduling follow-up appointments within the CFDT as well as with referring physicians, performing data entry, medical record documentation, and assisting with research as necessary.

What you will do

  • Responsible for ensuring postpartum follow-up is obtained on all postpartum patients that delivered in the Special Delivery Unit.
  • Contacts patients by phone or email to assess postpartum status.
  • Schedule follow-up appointments as needed, or refer for care based on routine, urgent or emergent needs.
  • Performs postpartum patient care coordination as requested.
  • Document encounters in the patient's medical record and make appropriate referrals for ongoing medical care, lactation support, and mental health services.
  • Provide support and education for patients in their postpartum needs.
  • Develop and maintain a clinical postpartum follow-up database and resource database.
  • Participate in research endeavors including data entry, literature review, database maintenance and representation of the CFDT/SDU at national conferences and the CHOP annual Fetal Family Reunion.
  • Coordinate postpartum plan of care with patient and providers.
  • Provide the patient and provide the clinical team with updates and pertinent medical information.
  • Obtain missing records/documentation needed to provide postpartum care, including sources from outside CHOP referring providers
  • Maintains and enhances database
  • Maintains database by updating patient demographic and clinical information regularly, accurately and independently.
  • Enters updated information following Postpartum visits in a timely manner.
  • Runs a variety of queries and reports from the database.
  • Maintains familiarity with the structure of the database and collaborates with Program personnel and Divisional computer specialists to enhance its scope and utility.
  • Clinical Responsibilities
  • Collaborate with medical staff and advance practice nurses in patient care activities when possible.
  • Assist in the development of case report forms.
  • With the assistant of the principal investigator and the Divisional administrator, investigates and resolves simple administrative questions about grant requirements and structure.
  • Education
  • Assist in developing education materials for patients and families if a specific need is identified.
  • Lecture to staff regarding program data and topics.

Education Qualifications

  • Associate's Degree Nursing Required
  • Bachelor's Degree Nursing Preferred

Experience Qualifications

  • At least one (1) year Labor and delivery, and women's health experience Required
  • Previous fetal diagnosis, treatment, and intervention experience Preferred

Skills and Abilities

  • Excellent clinical, organizational and communication skills.
  • Knowledge of and ability to use standard computer programs.
  • Knowledge of, or ability to learn, database programs, plus understanding of data management concepts.
  • Basic understanding of academic, research and/or clinical setting.

Licenses and Certifications

  • Registered Nurse (Pennsylvania) - Pennsylvania State Licensing Board - upon hire - Required
