Intensive Care Coordinator - Bachelor's Level
Exciting opportunity to join our innovative and growing children's mental health program as a Bachelor Level Intensive Care Coordinator!

Responsibilities:- Provide comprehensive care coordination to children and families.
- Perform initial clinical assessments to identify family and youth's need and goals.
- Develop, implement and monitor individualized care plans based on family needs and goals.
- Facilitate care plan team meetings.
- Act as a liaison with collateral agencies and programs
- Advocate for families and educate them on self-advocacy
- Identify and assist youth and families in accessing necessary services
- Ensure proper documentation, and safety planning
- Participate in staff meetings, supervision, and training

Required Skills
- Excellent communication and organizational skills
- Basic computer fluency, including Microsoft Office
- Valid driver’s license and reliable vehicle
- Bilingual in Spanish or Portuguese highly preferred
Required Experience
- Bachelor's degree in a mental health-related or human services field (such as social work, counseling, child development, psychology, human services) from an accredited college or university
- Minimum of one-year experience working with children with serious emotional disturbance
- Community-based experience strongly preferred
